discussion of Long Term Care needs identifies the best choice
of companies and programs.

AFSA member receives proposal from AFSA representative at The
Hirshorn Company.

AFSA member chooses coverage and options in discussion with
AFSA representative at The Hirshorn Company.

AFSA member completes and submits application to The Hirshorn
Company with check payable to the insurance company. Insurance
company receives application and check.

Insurance company representative calls AFSA member for a telephone
interview and in some cases arranges a face-to-face interview
appointment.

Insurance company requests additional information such as Attending
Physician Statements after AFSA member signs a form allowing
release of medical records.

Insurance company reviews application and additional information.
Insurance company approves AFSA member and issues policy, or,
company declines AFSA member due to health reasons.

AFSA member receives policy. AFSA member has 30 days to decide
if he or she will keep the policy. AFSA member returns policy
delivery receipt if accepting policy, or returns policy if he
or she no longer wants coverage.
