Group Accident Insurance Plan

A Personal Accident Insurance Program
(including medical evacuation and repatriation)

Designed specifically for the members
of the American Foreign Service Association.

Statistics published by the National Safety Council show that accidental bodily injuries are the fourth greatest cause of death in the United States and the number one killer of Americans between the ages of 1 and 45. These figures vividly point out the necessity for sound accident insurance protection. With this in mind, we have made arrangements with Federal Insurance Company to make personal accident insurance available under a group insurance program. Highlights of the program include:

  • Terrorism included at 100% of the limit of coverage purchased.
  • War risk included (except in the U.S., Canada, and Iraq) at 100% of the limit of coverage purchased.
  • The rate for this coverage is substantially lower than the cost of like insurance purchased individually.
  • The benefits provided are accidental death, medical evacuation and repatriation or dismemberment, including loss of speech and/or hearing.
  • The plan provides protection 24 hours a day worldwide, on and off the job.
  • Benefits are payable in addition to any other insurance you may have.
  • You may select any amount of insurance from a minimum of $50,000 to a maximum of $500,000 in multiples of $50,000.
  • You may also insure your spouse and children.
  • You can change amounts whenever you wish.

Eligibility
Accidental Death & Dismemberment (AD&D)
All AFSA members and their spouses (under age 70) and dependent children may apply for coverage. Dependent children means all unmarried children, including step-children, foster children or legally adopted children of the member over 14 days and under 19 years of age (26 if a full-time student in an accredited school or college).

Medical Evacuation & Repatriation
While you or your spouse and dependents are traveling outside the United States, you are covered in the event of a medical emergency.

Benefits
Accidental Death & Dismemberment (AD&D)
This coverage provides worldwide protection 24 hours a day, on and off the job. Coverage applies whether on land or water, traveling by common carrier or private passenger automobile. Even air travel in planes operated by the Military Air Transport Service of the U.S. or by a similar service of any country is covered.

You may select any amount of insurance from a minimum of $50,000 to a maximum of $500,000 in multiples of $50,000. You may change benefit amounts whenever you wish.

If as a result of accidental bodily injury the Insured suffers death, dismemberment, loss of sight, speech, hearing or the thumb and index finger within one year after the date of accident, the company will pay the following benefits:

Only one benefit, the largest to which you are entitled, is payable for all losses resulting from one accident.

AD&D losses due to War or Terrorism will be covered for 100% of the Scheduled benefit.

War Risk is not covered in U.S., Canada, and Iraq. War is defined as: 1) hostilities following a declaration of war by a government authority 2) if there is no declaration of war, then armed, open and continuous hostilities between two countries; or 3) armed, open and continuous hostilities between two factions, each in control of territory, or claiming jurisdiction over the site of the area of the hostility.

Medical Evacuation & Repatriation
While you or your spouse and dependents are traveling outside the United States, you are covered up to $75,000 in the event of a medical emergency.

If, as a result of accidental bodily injury, illness or disease an emergency Medical Evacuation and/or Repatriation is necessary, the plan will pay covered expenses up to a maximum of $75,000. The Medical Evacuation and/or Repatriation must be ordered by a physician and approved and coordinated by Medex Assistance Corporation.

Medical Evacuation means the emergency transportation from your location of injury or illness (outside the U.S.) to the nearest hospital where appropriate medical treatment can be obtained.

Repatriation means your transfer from that hospital where the emergency medical care was given to your country of domicile to obtain further medical treatment or to recover. Repatriation also means the necessary arrangements for the return of your remains in the event of death.

Covered Expenses means the costs for:
1) Most direct and economical transportation required to transport you during a medical evacuation.

2) Medical supplies and services which:
     a) are ordered by the attending physician
     b) are in the opinion of the attending physician,
         necessarily incurred in connection with the medical evacuation.

3) Necessary expenses for embalming, cremation, transportation and purchase of a shipping container.

Covered Expenses do not include those which occur while you are traveling against the advice of a physician and/or traveling for the purposes of obtaining medical treatment.

Exclusions
This insurance does not cover loss resulting from: intentionally self-inflicted injuries; suicide or attempted suicide while sane; illness, disease, normal pregnancy or resulting childbirth or miscarriage, and bacterial infection except bacterial infection of an accidental bodily injury, or if death results, from the accidental ingestion of a substance contaminated by bacteria or virus; accident occurring while the Insured is operating, learning to operate or serving as a crew member of any aircraft and loss while participating in military action in the Armed Forces of any country.

Term of Individual Insurance
For members who enroll, coverage will become effective on the first day of the month following the month the completed application is received by the Plan Administrator.

Coverage will continue in force until:
You reach the age of 85;
You discontinue participation in the plan;
The master policy is terminated, provided your premium is paid;

Coverage for Spouse and Dependent Children ends when the member's coverage stops.
